Please choose your purchase option and fill out the player registration form. Click on the links below to print out the forms required to complete registration. Every player is required to have the following items completed and bring all required items to field on August 1st.
Fees Paid in Full and Registration Form filled out
Copy of Birth Certificate for player file, please bring Original Birth Certificate to verify authenticity
Three (3) Copies of most current school report card (final grades only, progress reports do not qualify)
Official Pop Warner Physical Form, Filled out and signed by physician within one year of the August 1st season start date. (School Yellow Physical forms do not qualify. )
Registration Form and Fees
Please fill out registration form for each player in household
First Participant: $200 registration fee
Additional Participants: $25 discount = $175 registration fee
Equipment / Uniform Fees
Football: $50 equipment rental fee per household (refunded upon proper return of helmets, pads, and uniforms)
Cheer: $80 package per participant
